Prior to your upcoming appointment with us, we require all necessary documentation be completed and submitted by 5:00 pm one business day before your scheduled visit or your Provider will cancel your appointment and will need to be rescheduled.
This helps us ensure that everything is in order and allows us to provide you with the best possible care during your appointment. Please take a moment to review and complete all required documents. If you have any questions or need assistance with the documents, feel free to contact the office.
Please have the following completed:
Appointment confirmation email: You will receive an email to confirm your appointment. Click “Confirm Appointment” to let us know you will be at your scheduled time.
Consents: Please visit https://ifp.evahealth.com/login to sign any consents listed on your main page.
Invoice: You will receive an email with an invoice to pay your fee due before your appointment, this must be paid no later than one day prior to keep your appointment.
